Certain medical conditions can increase the risk of gums disease, which may also affect overall health. Delta Dental's Integrated Oral Health Option helps eligible members take a proactive approach to their oral health access to additional preventive dental care.

The Integrated Oral Health Option offers you up to four dental cleanings and/or periodontal maintenance procedures per benefit period instead of the usual two cleanings per year.


Am I eligible for extra dental cleanings?
You’re eligible if you have been diagnosed by your physician as having any of the following medical conditions:

  • Cardiovascular disease
  • COPD
  • Diabetes
  • Dry mouth related autoimmune diseases, medications, or menopause
  • Head and neck cancers
  • Kidney disease
  • Lupus
  • Pregnancy


What do I need to do next?
Fill out the Integrated Oral Health Option attestation form and include your provider's contact information. The completed form can then be faxed or mailed to:
