Box 16354 Little Rock, AR 72231 Most members won’t need to do anything since our participating dentists in New Jersey and Connecticut have already been notified of the change. Members who reside out of state or see an out-of-network dentist should inform their dental office of the change to reduce any possible delay in their claims. Members can also print an updated ID card with our new address through their MySmile account |
