About Delta Dental Patient Direct

1. What is Delta Dental Patient Direct?
Delta Dental Patient Direct is a dental membership program that offers access to a network of Patient Direct dentists who have agreed to provide dental care at fees no higher than the Delta Dental Patient Direct fee schedule. This is a program where people purchase access to a network of Delta Dental Patient Direct participating dentists. We provide access to this network for an annual membership fee, but you choose the dentist and your dental care program individually. This is not insurance coverage.

2. How does it work?
Dentists agree to accept up to the Delta Dental Patient Direct fee schedule as their full fee for services rendered. When you visit the Delta Dental Patient Direct participating dentist of your choice, and show your Delta Dental Patient Direct membership card at the time of treatment, you will be charged no more than the fee schedule for the respective service.

3. Is Delta Dental Patient Direct insurance?
No. Delta Dental Patient Direct is not insurance

4. How much does it cost?
Individuals pay an annual enrollment fee of $66.00. Families pay an annual enrollment fee of $96.00 (regardless of how many dependents are under 23 years old).

5. How can I pay for Delta Dental Patient Direct?
You may pay by MasterCard, Visa, or Discover credit/debit card or personal check (allow 2-5 business days for processing).

6. How do I sign up?
To sign up, go to www.patientdirectnj.com or print and complete the paper application.

Benefit Information

1. When will I get my membership card and other information?
Once your payment has been processed you will receive a membership card and packet by mail - usually within 7 to 10 days. If you do not receive your membership card and packet within 7-10 days, please call our Customer Service Department at 877-TOOTH-07 (877-866-8407).

2. What does the Delta Dental Patient Direct Family Plan cost, and who does it cover?
Your annual fee of $96.00 covers you, your spouse or domestic partner, and every dependent under the age of 23 years old.

3. Are there any procedures that aren't covered?
Most general dental services are included, such as routine cleanings, fillings, and x-rays, as well as crowns, root canals, dentures and orthodontia. To find out which services are not included, please call 877-TOOTH-07 (877-866-8407).

Dentist Information

1. Do I have to see a Delta Dental Patient Direct dentist?
Yes. You must see a Delta Dental Patient Direct provider to receive services at fees no higher than the Delta Dental Patient Direct fee schedule.

2. Can I use my New Jersey Delta Dental Patient Direct membership in states other than New Jersey?
No. If you join Delta Dental Patient Direct in New Jersey, you may only visit a New Jersey participating Delta Dental Patient Direct dentist. 

3. How can I find a Delta Dental Patient Direct participating dentist?
To identify a Delta Dental Patient Direct participating dentist in your area, please use the online dentist search on this Web site.

4. I have already started treatment with a dentist. Can I still sign up in Delta Dental Patient Direct?
This program does not apply to dental treatment which has begun prior to the date you joined Delta Dental Patient Direct.

5. I have a treatment plan from a dentist. Do I need to tell my dentist I have Delta Dental Patient Direct before I begin treatment?
Yes. You will need to notify the dentist that you have joined Delta Dental Patient Direct before you begin treatment.

1. Is there a cancellation policy?
At Delta Dental of New Jersey, the customer is the focus of our business. If you decide for any reason you are not satisfied with your Delta Dental Patient Direct coverage, please call the Customer Service number on your membership card (877-866-8407). You may obtain a complete refund within 30 days of joining.
