Useful forms include:

Student Documentation Verification Forms, which must be filed annually if you have a dependent who is a full-time college student in order to continue dental coverage.

Dental Claim Forms, which can be used when you or your dentist file a claim for dental treatment. (Of course, there's a good chance that your dentist won't require these forms. Most participating dentists keep Delta Dental claim forms on file. In addition, many participating and nonparticipating dentists now file their claims directly to us online, through our secure Web site.)
