subscribers: If I send an
e-mail to Customer Service, what type of information
should I include?
Please provide all pertinent information with your
inquiry, and be very specific. The subscriber's Social
Security number is important because it's how we
identify you in our system. If you prefer, you may
provide your full name, date of birth, and group number
rather than the subscriber's Social Security number.
Also, please include a daytime phone number so we can
follow up if necessary.
Please remember that, for security
reasons, you should use the Contact Customer Service
Form in the Benefits Connection area of our Web site
whenever sending e-mail messages with confidential
information to Customer Service. Information submitted
on this form is protected and encrypted, which means it
cannot be read by unauthorized persons as it travels
over the Internet.
When will I receive a response
to my e-mail?
Customer Service responds to e-mails within 24 hours.
E-mails received on weekends and holidays are responded
to the next business day.
Does Delta Dental issue ID
cards?
ID cards are not issued by Delta Dental (except for two
groups). If you are with another group and require an ID
card, you will need to contact your group's benefits
administrator.
How can I order claim forms?
Claim forms can be downloaded from our Web site, at
www.deltadentalnj.com. Click Forms on the navigation bar
on the left side of the screen. Please note that you may
not need a form. Your dentist can now file claims
directly to Delta Dental online. See the related story
in this issue.
How can I get information about
my benefits coverage or check my claim status?
Go to Benefits Connection, at www.deltadentalnj.com.
Click Subscribers and go to the Benefits Connection
Logon box. Please note that Benefits Connection is the
best way for you to determine what Delta Dental will
cover for each procedure. Customer Service cannot
predetermine through an e-mail exactly what will be paid
for a procedure.
