Improper Submission of Claim Forms
 

It is improper to:

  • Let a patient convince you to submit an inaccurate date of service on a claim form for any reason.

  • Let a patient convince you to submit an inaccurate procedure on a claim form.

  • Let a patient convince you to omit reporting other insurance carrier coverage or payments on a claim form.

  • Waive a patient's copayment responsibility without legibly and conspicuously disclosing that intention on the face of the claim.

  • Fail to reflect any discounts or coupon use on the face of the claim.

  • Fail to correctly identify both the billing dentist and the treating dentist's name and license number.

  • Submit for services performed by non-licensed individuals.

  • Submit claims for services performed by non-par dentists in your practice under your par license number.

Any of the above improprieties will result in a violation of the Delta Dental of New Jersey Participation Agreement and may also result in the violation of:

  • The New Jersey Insurance Fraud Prevention Act

  • Other State and Federal Laws
