To streamline the claim submission process, Delta Dental has developed an enrollment form for acceptance of signature on file for manually submitted paper claims, as well as electronically submitted claims.

For Delta Dental to accept claims without patient signatures, the authorization form requires you to certify that you maintain the patient’s signature on file for claim submission and release of related information.

To take advantage of signature on file for all claims, you must complete and sign the Signature on File Form and mail it back to the Professional Relations Department.
